Esperanza Health Centers' Nurse Practitioner Postgraduate Training Program is a 12-month program that provides intensive training to both the clinical complexity of family practice in the safety net setting, and trains to a high performance, PCMH model for primary care.

The program is specifically intended for new nurse practitioners who are committed to developing careers as primary care providers in the challenging setting of community health centers and other safety net settings.

The Postgraduate Training is a full-time, 12-month salaried position with the opportunity for a one-year employment commitment after completion of the program. The program structure includes precepted primary care sessions, specialty rotations, mentored independent clinics, didactic sessions and quality improvement project.

About the Nurse Practitioner Postgraduate Training Program

We’re training postgraduate nurse practitioners to Esperanza Health Centers’ model of high-performance health care: advanced access scheduling, planned care, the chronic care model, integrated behavioral health/primary care, team-based care, with expert use of health information technology and the electronic health record.

Six (6) nurse practitioner or adult/gerontology nurse practitioner fellow positions will be available for the 2023-24 cohort. Our next Nurse Practitioner Postgraduate Training Program will begin the summer of 2023.

The Postgraduate Training year begins with intensive orientation to Esperanza Health Centers,  the neighborhoods in which the training sites are located, and the health problems of the target population, and includes walking tours of neighborhoods, tours of Esperanza Health Centers sites statewide, meetings with community leaders, technical training for electronic health record, and orientation by our chief medical officer to the responsibilities and privileges of medical staff participation at Esperanza Health Centers.


The Postgraduate Training has five key components:

Precepted “Continuity Clinics”
(4 sessions/week) These are the cornerstone of the Nurse Practitioner Postgraduate Training Program. In precepted clinics, the residents develop their own patient panel while having an expert Esperanza Health Centers primary care provider (MD or APRN) exclusively assigned to them.

Specialty Rotations:
(2 sessions/week for 1 month) Specialty rotations in areas of high-volume/high-burden/high-risk situations most commonly encountered in the setting of the FQHC. Rotations may include but are not limited to: orthopedics, dermatology, women’s health, pediatrics, geriatrics, newborn nursery, HIV care, adult behavioral health, child and adolescent behavioral health, endocrinology, and MAT.

Mentored Clinics:
(2 sessions/week) During mentored clinics, the postgraduate trainees work as a member of a team,  and see patients at the delegation of the primary care providers, who remain available for consultations. The focus is on the practice of episodic and acute care visits.

Didactic Education Sessions:
(1 session/week) Formal learning sessions on a variety of complex clinical challenges most commonly encountered in FQHCs. The content of the presentations is planned to correspond to the residents’ current clinical experiences.

Program Goals:

The Nurse Practitioner Postgraduate Training in family practice and community health has the following goals:

  • Nurse Practitioner Photo 2Increase the number of NPs ready to serve in leadership roles in community health settings.
  • Improve the clinical skills, confidence, productivity, and job satisfaction of new NPs who choose to work in underserved community settings.
  • Provide new NPs with the necessary clinical training to be able to serve as primary care providers in the complex setting of the country's FQHCs.
  • Increase access to quality primary care for underserved communities by training NPs in a Federally Qualified Health Center-based residency program.


Applicants must be recent graduates (no more than 24 months) from an accredited master's or DNP program, licensed (IL APRN) and credentialed (FNP-C) or license/credential-eligible as nurse practitioner or adult-gerontology nurse practitioner, fluency in Spanish required, with a stated commitment to practice as a primary care provider in a Federally Qualified Health Centers (FQHC) or other safety net settings.


Compensation Package Updates

  • Salary increase to $80,000.
  • Additional $2,000 in CME funds for the year.
