Esperanza Health Centers is pleased to announce the launch of a Diabetology Fellowship based in Chicago, Illinois. This fellowship is a community-based training experience housed within a federally qualified health center (FQHC) serving predominantly low-income and underserved communities on the southwest side of Chicago.
The program is designed for physicians seeking advanced training in diabetes care within a high-need, real-world clinical setting that emphasizes health equity, continuity of care, and interdisciplinary collaboration.
Curriculum & Training Experience
Fellows will train primarily at Esperanza’s Brighton Park North location and gain comprehensive experience through:
- Dedicated diabetes clinic with complex case management
- Continuity clinic within a primary care setting
- Interdisciplinary care model including medical, nursing, behavioral health, and support services*
- Exposure to culturally responsive and linguistically appropriate care
The fellowship emphasizes hands-on clinical learning, team-based care, and population-health approaches to diabetes management. Spanish-language proficiency is required due to the patient population served.

- Completion of an accredited medical residency program
- Strong interest in diabetes care and community-based medicine
- Commitment to caring for underserved populations
