Diabetology Fellowship

Esperanza Health Centers is pleased to announce the launch of a Diabetology Fellowship based in Chicago, Illinois. This fellowship is a community-based training experience housed within a federally qualified health center (FQHC) serving predominantly low-income and underserved communities on the southwest side of Chicago.

The program is designed for physicians seeking advanced training in diabetes care within a high-need, real-world clinical setting that emphasizes health equity, continuity of care, and interdisciplinary collaboration.

 

Curriculum & Training Experience

Fellows will train primarily at Esperanza’s Brighton Park North location and gain comprehensive experience through:

  • Dedicated diabetes clinic with complex case management
  • Continuity clinic within a primary care setting
  • Interdisciplinary care model including medical, nursing, behavioral health, and support services*
  • Exposure to culturally responsive and linguistically appropriate care

The fellowship emphasizes hands-on clinical learning, team-based care, and population-health approaches to diabetes management. Spanish-language proficiency is required due to the patient population served.

 

Eligibility & Expectations

  • Completion of an accredited medical residency program
  • Strong interest in diabetes care and community-based medicine
  • Commitment to caring for underserved populations
  • Spanish-language skills required
