Having chronic care needs doesn’t mean you can’t live at home and thrive in the community you love. Panorama - Esperanza's Program of All-Inclusive Care for the Elderly (PACE) - offers an alternative to nursing home care and provides seniors with the services and support they need to age in their own community.

PACE aims to improve the quality of life of seniors who need ongoing attention and care, but don’t want to lose the independence that comes with living at home. To be eligible, seniors must be:

  • 55 or older
  • certified by the state to need nursing home care
  • able to live safely in the community at the time of enrollment, and
  • living in our PACE Zip codes: 60453, 60455, 60456, 60458, 60459, 60501, 60609, 60629,
    60632, 60636, 60638, 60652

Senior at PACE Center

Participants live at home but get transportation to Esperanza’s Panorama PACE Center where they receive services for their medical, wellness, and social needs, including:

  • adult day care offering nursing; physical, occupational and recreational therapies; meals, nutritional counseling, social work and personal care
  • medical care provided by a PACE physician
  • home health care and personal care
  • all needed prescription drugs
  • social services
  • medical specialties like audiology, dentistry, optometry, podiatry and speech therapy
  • respite care, and
  • hospital and nursing home care, when necessary

If you are enrolled in Panorama, Esperanza will become your sole service provider.  This means that you must receive all needed health care, including primary care and specialist physician services (other than emergency services) from Esperanza or an entity authorized by Esperanza. Please note that Esperanza guarantees your access to services, but not to a specific provider. You may be fully and personally liable for the costs of unauthorized or out-of-network services.

Your Panorama Team

Our interdisciplinary team works together to ensure that participants get the attention and care they need at home and at our Panorama PACE center. Your team includes:

  • Primary Care Provider
  • Registered Nurse
  • Masters Level Social Worker
  • Physical Therapist
  • Occupational Therapist
  • Recreational Therapist/Activities Coordinator
  • Dietician
  • PACE Site Director
  • Home Care Coordinator
  • Personal Care Attendant or representative
  • Driver or his/her representative

 

Paying for Panorama

What you pay for PACE depends on your eligibility under Medicare and Medicaid*.

  • If you are eligible for Medicaid, or dually eligible for both Medicare and Medicaid, you won't pay a monthly premium for PACE.
  • If you don't qualify for Medicaid, but you have Medicare, you'll be charged a monthly premium for PACE.
  • There's no deductible or copayment for any drug, service, or care approved by your health care team.
  • If you don't have Medicare or Medicaid, you can pay for PACE privately.
*A potential PACE enrollee may be, but is not required to be, any or all of the following: a) Entitled to Medicare Part A, b) Enrolled under Medicare Part B, or c) Eligible for Medicaid.

 

Enrolling in Panorama

The first step will be to meet with you at your home to learn more about your health and support needs, and to answer any questions you might have about the program, our services, and possible costs. We'll conduct an assessment of your health and eligibility, and, if you qualify and want to proceed, we'll submit your application to the state for its approval. Our team will create a personalized care plan that meets your needs and we'll review it in depth with you.  At any point in this process, we'll invite you to come visit our Panorama PACE Center so you can see for yourself what it is like and meet other members of our team.

Once you've signed the agreement, You'll start enjoying the benefits of Esperanza's Panorama PACE program on the first day of the following month.

Disenrolling from Panorama

You can choose to disenroll from Esperanza's Panorama PACE program at any time.  We'll coordinate the disenrollment process with Medicaid and Medicare (as applicable) to make sure any transitions in your care happen as seamlessly as possible.  In rare occasions, a participant may have to be disenrolled because they've moved out of our approved service area, they displayed threatening or disruptive behaviors, or because of changes in our ability to deliver care.  In these instances, the state will review and make a final determination about a participant's disenrollment.

If you need to file a grievance

We want to make sure that you're satisfied with the care that you receive. If you have a concern about your care or believe that you are not receiving the services you need, please let us know right away - either verbally or in writing - so that we can address your concern.  You, a family member, or your legal representative can file a grievance about any aspect of your care within a year of its occurrence, and we'll promptly investigate and seek a resolution.  If you want to submit a grievance in writing, please send to:

Your right to appeal

If Esperanza denies your request for a service, for payment of a service, or if you are involuntarily disenrolled from the program, you have the right to file an appeal either by calling us directly, by meeting with us directly or by sending a letter to:

The PACE Medical Director and an impartial and appropriately qualified person will promptly review your appeal no later than 30 calendar days after receiving it.  In certain situations, you may request an expedited review process and we will review your case immediately.
