Sabor y Salud

Sabor y Salud is a new program that utilizes food as medicine to address dietary needs that can have an impact on chronic disease.  Patients and community members can receive a FREE weekly box of fresh organic fruits and vegetables.  There are no requirements to participate in the program.

Reach Out and Read

Reach Out and Read integrates reading into pediatric care to help families build meaningful bonds and promote healthy brain development.  Books are given to children during their Well-Child Visits starting at 6 months old until the age of five. Reading to children can positively impact their language and listening skills while nurturing their curiosity and memory.

Dolly Parton's Imagination Library

In partnership with Open Books and The Dolly Parton Imagination Library Program, Esperanza Health Centers will continue to promote literacy among our youngest patients. Every month, children will be gifted one age-appropriate book from birth until the age of five. Reading aloud consistently promotes healthy child development and prepares the child for success in school and beyond.

Prenatal programming

 

CenteringPregnancy

CenteringPregnancy brings pregnant patients out of the exam room and into group settings for their care.  The main components of group prenatal visits are Health Assessments, Interactive Learning and Community Building.  Learn about the topics that matter most to parents and their babies.


Other programs

 

Immigration Physicals

Immigration physicals are typically required for anyone who applies for lawful permanent resident status. Esperanza is an approved site.

Requirements:

  • Government-issued photo ID from the U.S. or your country of origin (such as a valid passport or driver's license)
    • If you are 14 years old or younger, bring identification that shows your name, date of birth, place of birth, and parents' full names. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
  • Immunization records
  • Payment
    • Physical exam (includes lab work and TB test) is $175. Vaccines will be charged separately depending on which vaccines are needed.
